Tenecteplase (TNK) Dosing for Ischemic Stroke Calculator
Doses tenecteplase (a type of tPA) for use in ischemic stroke.
This calculator is for use in ischemic stroke patients only. Do NOT use for calculating doses in STEMI or PE.
Advice
- Administer TNK when indicated and when TNK is the institutionally preferred formulary thrombolytic for ischemic stroke.
- 5 mL (equivalent to 25 mg) is the maximum dose.
Management
- Following TNK administration, blood pressure and neurological assessments should occur every 15 minutes for 2 hours, then every 30 minutes for 6 hours, then hourly until 24 hours following TNK treatment.

Following TNK administration, maintain blood pressure <180/105 mmHg. The use of parenteral antihypertensive(s) may be necessary to achieve this goal blood pressure.
Critical Actions
- If the patient develops acute hypertension, worsening neurological exam, severe headache, nausea, or vomiting, obtain an emergency head CT.
