  • This tool should not replace clinical judgment; results should be used alongside patient history, physical examination, and other assessments to guide treatment discussions.
  • Combined or individual hematologic and nonhematologic scores may be used to stratify a patient’s risk for severe chemotherapy toxicity.
  • A higher-risk score should not automatically preclude treatment; patient goals and quality of life remain central considerations.
  • For those at a higher risk for toxicity:
    • Consider dose modifications, alternative regimens, or supportive measures.
    • Address modifiable risk factors such as nutritional status and blood pressure.
    • Collaborate with nutritionists, physical therapists, social workers, and other support services to optimize patient care.
