This tool is intended for risk assessment and screening purposes. Patients with an elevated score or symptoms of diabetes should undergo formal diagnostic testing.

Management

Score ≥5: 

  • Patient is at high risk for type 2 diabetes.
  • Recommend confirmatory testing (e.g., fasting glucose, HbA1c).
  • Counsel on lifestyle modifications, such as diet and exercise, to reduce diabetes risk.
  • Consider referral to diabetes prevention programs or nutrition specialists, if available.

Score <5: 

  • Provide general advice on maintaining a healthy lifestyle.
  • Reassess risk periodically.
  • Keep in mind that the patient may still be at risk for type 2 diabetes.
