Age >65 years
Cardiovascular disease in clinical history
Structural heart disease, CHF, PAD, stroke/TIA
Syncope without prodrome
Abnormal EKG
Arrhythmia, AV conduction disorders, BBB, conduction delay, LV or RV hypertrophy, left axis deviation, old MI, ST segment or T wave abnormalities related to ischemia

Advice

This tool complements, but does not replace, clinical judgment, a careful history, and a thorough evaluation of syncope patients in the ED.

Management

Low risk (score 0–1):

  • Consider further outpatient evaluation and follow-up.
  • Provide patient education on warning signs and when to return to the ED.

Intermediate to high risk (score ≥2):

  • Consider hospital admission for further monitoring and investigation (e.g., telemetry, echocardiography).
  • Initiate consultations with cardiology or neurology as needed.
