Modified Brain Injury Guideline (mBIG)
Defines the management of traumatic brain injury.
Use in traumatic brain injury patients with intracranial hemorrhage, Glasgow Coma Score 13-15, and no focal neurologic or pupillary findings.
Management
mBIG 1:
- No hospital admission.
- 6 hr ED observation.
- No repeat head CT.
- No neurosurgery consult.
- Q2 neurological assessment.
- Trauma team assessment at 3 hrs.
- Attending assessment at 6 hrs.
- GCS 15 for discharge.
mBIG 2:
- Hospital admission to general medical floor for 24-48 hrs.
- No repeat head CT.
- No neurosurgery consult.
- Q2 neurological assessment.
- Trauma team assessment at 3 hrs.
- Attending assessment at 6-12 hrs.
- GCS 15 for discharge.
mBIG 3:
- Continue with standard of care at your institution.
