Predicts 30-day mortality or serious adverse events (MI, intubation, etc) in ED patients with COPD.

History

Coronary bypass graft
Intervention for peripheral vascular disease
Intubation for respiratory distress
Any history

Examination

HR ≥110/min
On arrival to ED
Too ill to do walk test after treatment in ED
SaO2 <90% or HR ≥120/min after 3 minute walk test (see Evidence for details)

Investigations

Acute ischemic changes on EKG
By clinician judgment
Pulmonary congestion on chest x-ray
By clinician judgment
Hemoglobin <10 g/dL (100 g/L)
Urea ≥12 mmol/L (BUN ≥34 mg/dL)
Serum CO2 ≥35 mEq/L (35 mmol/L)

Consider local practices when making disposition decisions. The study was conducted at 6 Canadian hospitals; in Canada, approximately 37.5% of emergency patients with COPD exacerbation, compared with 80% in the US (Stiell 2014).
